Privacy Policy

Your privacy is important to us. This Privacy Policy describes how we intend to collect, use and secure your information in connection with your use of CareKraft’s services. Terms like “we” and “us” refer to CareKraft. Capitalized terms that are used in this Privacy Policy but not defined, shall have their meaning assigned to them in the Terms of Use. When you use our Services, you agree to our policies on the collection, use and disclosure of your information as explained in this Privacy Policy.

1. Information Collected by Our Services

While providing our services, we collect information from you in various different ways.

We may collect the following information from you:

1.1 Personal Information

We may require you to provide us with certain information that identify you personally (Personal Information), including:

  1. Contact Data (such as your address, phone number, e-mail address)
  2. Demographic Data ( such as your gender, date of birth and zip code)
  3. Insurance Data ( your insurance plan and insurance carrier)
  4. Medical Data ( such as the names of the previous heath care providers visited, the purpose of your visit and the date of visit)
  5. Other relevant information you would like to share with us.

If your chosen mode of communication is e-mail or letter, any information provided in such communication may be considered and collected as Personal Information.

1.2 Activity

Information associated with you and your use of the Services, such as your browser type, IP address, unique device identifier, the phone calls that you make through the use of the Services, requesting URL, referring URL, browser language, the pages viewed by you, and the date and time of your visit may be collected and reserved.

We may also use third party analytics services in association with the Services to record mouse clicks, mouse movements, scrolling activity, and/or clicks to call or contact heath care providers through the use of the services. However, the information collected by the use of these services merely aim at identifying usability problems and make the Services even more convenient to use. We only intend to gather anonymous user information, and the recordings are stopped before you sign-in or create an account. You shall be notified in advance should we decide to use any third party analytics services to collect Personal Information.

1.3 Cookies and Similar Mechanism

We may use cookies, web beacons, local shared objects and other similar technology in connection with your use of the Services. Web cookies refer to a small piece of data transferred to your computer’s hard drive that contain information such as user ID, user preferences, lists of pages visited and activities performed while browsing the Services. While you have the option to block or delete cookies at your own expense and responsibility, you may lose access to some of the features of the Services on doing so.

1.4    Information Provided on Behalf of Children

In accordance with the Children’s Online Privacy Protection Act (“COPPA”), we do not collect information from children under the age of 13. If you are a legal guardian or parent of a minor child, you may use the Services on behalf of such child, keeping in compliance with the Terms of Use. Individuals under the age of 18 are not allowed to use the Services.

 

2.Information You Submit

The information you submit through the use of the Services may be stored by us. The information is used by us to fulfil your requests, provide Service functionality, improve Service quality, put up relevant advertisements, provide customer service, send out messages to you, support our systems, allow for disaster recovery, personalize your experience with us and be in line with legal obligations. While submitting information to or through the use of the Services, please note that:

  • You need to provide a valid e-mail address in order to sign up for an account through the use of the services, in addition to providing us with your full name, phone number, address and insurance data.
  • Information that you may share in a rating or review posting or public forums is no way private, and is deliberately open to the public. Disclosing any Individually Identifiable Health Information in public forums is not permissible. What you may reveal or write may be viewed, collected and used by third parties in ways that cannot or predicted or controlled by us.
  • You can invite friends, colleagues, health care providers and others whom you know ( Associates), and would like to join the Services, either by providing us with their contact information or by giving us the permission to use your address book from your mobile device, computer or other websites or services. We may also suggest Associates whom you may want to invite to use the Services, by the use of publicly available information. The invitation will provide your Associate the facility of opting out of receiving any further invitations to join CareKraft.
  • If at any time we include messaging into the Services, the messages you may exchange with others through the Services may be stored in order to be delivered and managed by us.

 

3.Third Parties

We refrain from renting, sharing, selling or trading Personal Information with third parties to serve their direct marketing purposes. However, we do work with third parties, and they may have access to some of your information.

3.1 Information Sharing

We may share Demographic Data with advertisers and other third parties, but only on a non-personally-identifiable basis. Also, your Contact Data, Demographic Data, Insurance Data and Medical Data can be shared with health care providers you choose to schedule through the use of our Services. We also disclose Personal Information and Traffic Data with our business partners who work with us to perform core services in connection to the operation of our Services. However, those business partners are liable to stick to the same standards of security and confidentiality that we have promised to you in this privacy Policy.

3.2 Service Providers

If we may need to outsource some of our technical and customer support, quality assurance testing, tracking and reporting functions, payment processing operations and other services to third parties, we may share some of your information to help them perform their services better.

3.3 Business Transfers

We may share information collected from you with subsidiaries, joint ventures and other companies under common control. However, they will be required under all circumstances to honor this Privacy Policy. If CareKraft is acquired by another company or all of our assets, the company will obtain all the information we possess, and will assume the rights and obligations in connection to those information, unless you are notified by us otherwise.

3.4 Health Care Providers

Health care providers, their agents and their employees should be thoroughly aware of their obligations to maintain patient confidentiality in addition to their obligations under the Health Insurance Portability and Accountability Act (“HIPAA”), both in terms of communicating with CareKraft and in responding to a service review posted through our Services. CareKraft does not accept any obligation of confidentiality in connection to communications that are not stated in this Privacy Policy or CareKraft’s Terms of Use.

 

4. Use and Disclosure of Your Information

We collect and store Personal Information with the aim to provideyou a more efficient, substantial and personalized experience. For example:

  • We may use your Personal Information to make our Services easier to use by not requiring you to enter the same information more than once or develop content that are most relevant to you or inform you about new products and services that we offer.
  • We may use your Contact Data to communicate with youwhenever required, such as to inform you about CareKraft products or services or to remind you of upcoming follow-up or appointments.
  • Your Demographic Data, Traffic Data, Insurance Data or Medical Data may be used to help you enjoy a more customized and tailor-made experience through the use of our Services.

You agree that CareKraft may need to use your Personal Information to grant your health care providers the permission to make appointments with other doctors on your behalf, and as required. Booking an appointment will be the sole purpose, refraining your health care providers from transmitting any information regarding the provision of health care.

Changing and Deleting Your Information

As a registered User of the Services, you have the right to change your username by signing in or modify some of the Personal Information included in your profile. On your request, CareKraft will make their best efforts to delete your account and all the Personal Information in your profile. However, some residual information may be retained by CareKraft even when your account is removed. When you delete your account, you need to understand that removed content may continue to exist in backup copies for quite some time. However, they will not be available to others. Moreover, information about you that has been shared with others or have been independently uploaded or retained by other Users, will continue to remain on the Services.

Security

We understand the importance of securing your Personal Information. We follow industry standards in protecting the Personal Information you have submitted to us, both during its transmission and upon receiving it. However, even though we make best efforts to store your Personal Information in a secure environment, hidden from the public, you must understand that the concept of fool-proof security is a myth. We do not guarantee to prevent the possibility of any unintended disclosure of your Personal Information. However, if we come to know of any such possibility or incident, you will use reasonable efforts to notify you of the same with immediate effect.

Updates and Changes to Privacy Policy

We may need to revise or modify this Privacy Policy from time to time. The use of your Personal Information will be governed by the most updated version of the Privacy Policy. We will notify you by e-mail or by posting a notice on the Services if any material changes are made to this Privacy Policy.
